Target: More than 600 people with disabilities many of whom suffer from physical, sensory problems and epilepsy.
Description: Community-based Rehabilitation (CBR) . The aim is to provide home care or community care to Persons with Disabilities by local people (belonging to the community of the patient) and as far as possible using local resources. The project now employs: 17 social agents, including 2 part-time, a coordinator, two physiotherapists and a nurse and ophthalmologist part - time. Their tasks are to: - Identify persons with disabilities - to advise the disabled person and his family - follow patients in their homes after the medical intervention. Everyone is working in close collaboration with other specialists, to help them make a treatment plan for each patient.
